HOW TO FAX THE DOCUMENT AND ORDER DOCUMENT REVIEW:

Steps: (Count, Click and Fax)

1. Review the online representation page on this website
2. Fill out and print this fax cover sheet with your name, address, telephone number, email address, fax number and your question from the above list on the cover page
3. Count how many pages your document has
4. Select the document review service option below which is based on the total number of pages of your document
5. Click the PayPal button in that box and follow the PayPal instructions
6. Write down the PayPal Transaction ID that was assigned to your order in the subject line of your fax cover sheet
7. Fax the cover sheet along with your document to (419) 831-3376, and the attorney will review your document and give you a response to your email and/or fax number that you provided.

Please do not send part of a document. If an incomplete document is sent for review, it will be returned to you along with a refund and will not be reviewed. Make sure the document is legible and that you are not submitting a copy of a copy where part of the document is cut off.
